Healthcare for the Homeless Houston's mission is to promote health, hope, and dignity for those affected by homelessness through accessible and comprehensive quality care.
Our vision is to provide pathways for moving beyond the cycle of homelessness through integrated healthcare and collaborative partnerships.

About

For more than 25 years, Healthcare for the HomelessHouston (HHH) has served as the city’s sole Federally Qualified Health Center dedicated exclusively to individuals and families impacted by homelessness. Guided by our mission to promote health, hope, and dignity, we deliver comprehensive, trauma-informed care that stabilizes health and supports pathways to housing and long-term stability.

In 2025, HHH served more than 6,000 Houstonians, delivering over 20,000 clinical and wraparound encounters through our integrated model of care, including:  

    1. Medical, including primary care, and specialty services such as podiatry, ophthalmology and dermatology  

    2. Mental health and psychiatric care, including telepsychiatry

    3. Substance use treatment, including Medication-Assisted Treatment (MAT)

    4. Case management and supportive services, connecting patients to housing, benefits, food access, and identification restoration

    5. Comprehensive dental care

    6. Point-of-care access to medications, to support chronic and psychiatric disease management  

    7. Outreach programs, including Project Access transportation, Jail In-Reach, and our Street Medicine Program

HHH’s care reduces preventable crises, sustains engagement in care, and strengthens health stability across Houston’s homeless response system.
